Payment and enrollment/waiver form must be received in order for your child to participate on The Monkey BUS. Prorated amounts will NOT be given for late sign ups (when paying the Seasonal Block). Current enrollment/waiver forms must be kept on file. If the bus is unexpectedly down due to an issue on my end, I will schedule a make-up day and if a make-up class cannot be scheduled I will issue a credit for one class in the upcoming Seasonal Block. NO refunds will be given for missed classes due to absences, childcare activities, childcare closures or behavioral issues. You may pay with cash or credit card at themonkeybusiness.net. If paying with cash, please put exact change in an envelope with your child’s name on it. Overpayments will credit to your child’s account. I have read and understand the policies and payment process above. I give permission for the childcare center to release my child to the The Monkey BUSiness Activity Bus Instructor Miss Stacey for class. I will not hold the childcare center responsible for activities held on The Monkey BUSiness Activity Bus. As with any physical activity, accidental injury could occur. Parents assume all responsibility for injuries due to participation on The Monkey BUS. Parents, please read the Concussion Awareness Form provided. In the event of any injury, The Monkey BUS will submit an incident report to the center. The Monkey BUS Staff reserve the right to remove any child due to behavioral disruptions and a refund will not be issued. Unless specified we reserve the right to use any and all pictures for promotional purposes on, but not limited to, program flyers, The Monkey BUSiness Activity Bus Facebook Page and our website.
